This error message appears when a file is uploaded to Site administration>Courses>Upload courses and the file is either:

an incorrectly formatted .csv file, in which case, please check Upload courses

or

a SCORM package, (such as from Captivate) See SCORM settings

or

a completely different file such as an .mbz or .tgz file (a Moodle course backup)
